I'm Receivng an Error Message When Applying for My 401(play) MasterCard

Upon your first 401(play) mobile app sign-in, you will be prompted with the screen "Set up 401(Play) Card". Follow this process and input your information as requested.
If, after the submission of your personal details, you receive an error message, including an "additional information required" warning, you will need to provide additional information to our MasterCard issuing partner, Juice Financial.
This can happen for one of more of the following reasons:
- Name change (marriage/divorce, identity, etc.)
- SSN change
- Previous or active fraud or identity theft instances, including data breaches
- Credit freezes applied by any credit bureau
- Multiple residence changes
- Foreign nationality
The good news is that this is easily resolved - simply review the ID requirement document pictured below ⬇️ (or download from the link above ⬆️), then gather up two identity documents (one from each column) in order to validate your account, and then decide if you’ll e-mail or fax those over to our financial partner, Juice Financial (e-mail/fax info is on the form).
If sending by e-mail, your document files are secured upon receipt and both faxed and e-mailed versions are destroyed after the verification process. When e-mailing, you may send color files in compressed JPEG, TIFF, PNG, and PDF formats.
One received, our financial partner will update us regarding your account status, and should additional information be required, you will be notified by us or Juice. Your updated account will become active once you activate your new 401(play) MasterCard, and you'll be able to use it immediately for booking travel, buying vacation clothes, or putting tires on the car for your long road trip!
